Help, sick ST first gear

All you ST gurus out there. My ST is sick . My first gear sounds horrible. It sounds like gears are grinding. Once past first, then it runs smoothly. Any thoughts before I take it to the shop and get reamed? :blink:

Sorry bout that. 2002, Gen 2, automatic 129706 miles. The noise is coming from the engine compartment while sitting in the drivers seat. No noise while in park or neutral. Would the serpentine have anything to do with it?
